Record a finding and action plan
From a test, add a finding with its problem statement, type, severity, affected assets, and remediation. Review it, then the auditee completes the root cause and corrective action plan.

Capture a finding from a test, set its type and severity, and track the corrective action plan.
Step-by-step
- 1 In the engagement, open the 'Tests' tab and open a test's 'Findings' action, then add a finding.
- 2 Enter the 'Title' and the 'Problem Statement' (for example, missing documented sign-offs on 45% of capital disbursements).
- 3 Use the evidence upload link to attach supporting files (xlsx, png, jpg, docx, pdf, up to 5).
- 4 Set the 'Type': Observation, Non-Conformity, or Opportunity for Improvement.
- 5 If the type is Non-Conformity or Opportunity for Improvement, set the 'Severity', and for a Non-Conformity set the 'Remediation Due Date'.
- 6 Add affected assets with 'Add Affected Asset' and recommended fixes with 'Add Recommendation'.
- 7 Review the details and save. The finding appears in the Findings list.
- 8 When the auditee responds, they complete the action plan: 'Root Cause Analysis', a corrective action title and scope, supporting documents, completion date, responsible personnel, and status.
